Using Evidence to Support Practice Change Initiatives: Case Resolution Meetings

This brief describes POC's formative evaluation of the early stage of the Case Resolution Meeting (CRM), a practice innovation that brings key players in a case (including legal parties) together to identify barriers and develop pathways to permanency for children who have been in care longer than 15 months. Overall, the evaluation found that stakeholders in each of the involved counties felt that the CRM is a useful process that has fostered collaboration and mutual problem solving 'fiercely" focused on permanency for children in long term care.
