This legislation will provide planning grants and technical assistance to state, local, and tribal governments to help them devise innovative, evidence-based approaches to coordinate services and improve outcomes and cost-effectiveness for those participating in Medicaid. The legislation will direct the Secretary of Health and Human Services (HHS) to convene a technical advisory council on the social determinants of health. The council will include program experts, state and local government officials, and community-based organizations. Up to $25 million in grants will be made available to state, local, and Tribal governments to develop Social Determinants Accelerator Plans, which target high-need Medicaid patients, identify key outcomes to be achieved through improved coordination of services, and includes a plan for linking data across programs to measure impacts of the new approach. The council will provide technical assistance to grantees.
